Sports Physical/Immunization Schedule

The Health Department Mobile Unit will be coming to Timberlake HS to provide immunizations and no cost physicals for Timberlake students.

1. July 18, 2:00-4:00 Sports Physicals ONLY
2. August 3, 8:00-2:00 Sports Physicals and Immunizations
(HS Science classroom next to library during enrollment)

Sports Physical Requirements

Sports physicals will be done at no cost. If parents can't attend, parents must complete all paperwork and provide explanations on any questions that they answered YES to on the questionnaire. The following sports physical forms MUST be completed by the parent or guardian:

Immunization Cost Questions:

The Oklahoma State Department of Health participates in the Vaccines for Children (VFC) Program, which allows our Health Departments to provide immunizations free of charge to low-income individuals, Native Americans, or those whose insurance does not cover the cost of immunizations.  

Which children are eligible? 

  1. Children through 18 years of age who meet at least one of the following criteria are eligible to receive VFC vaccine: 

  • Medicaid eligible: A child who is eligible for the Medicaid program. (For the purposes of the VFC program, the terms “Medicaid-eligible” and “Medicaid-enrolled” are equivalent and refer to children who have health insurance covered by a state Medicaid program) 

  • Uninsured: A child who has no health insurance coverage 

  • American Indian or Alaska Native: As defined by the Indian Health Care Improvement Act (25 U.S.C. 1603) 

  • Underinsured 

Children whose health insurance covers the cost of vaccinations are not eligible for VFC vaccines, even when a claim for the cost of the vaccine and its administration would be denied for payment by the insurance carrier because the plan’s deductible had not been met. 

We accept private insurance for payment of adult and child immunizations. If you have insurance, please bring your insurance card or a copy of your insurance card with their child’s immunization forms. A claim will be submitted to their insurance company for reimbursement, and you will be notified with a statement from OSDH of your responsibility for any co-pays, deductibles, or charges that are denied by your insurance company. Some sliding scale fees may apply. PAYMENT IS NOT REQUIRED TO RECEIVE SERVICES.
